The ED registration clerk checks the facility master patient index (MPI) and either updates information if the patient has previously been treated at the facility or enters the information if this is the patient’s first visit. The triage nurse takes a medical history and performs an initial clinical evaluation. This information is entered into a computerized ED encounter form and an automated triage system categorizes the patient. The triage systems assign an available ED bed in the bed assignment system. The patient is put on the schedule to be seen by an ED physician. The ED scheduling system tracks a patient in the ED and captures information on diagnosis, treatments, provider interaction, and amount of time a patient spends in the ED. An ED physician sees the patient by schedule order. The physician performs an examination, orders tests, writes prescriptions, makes a diagnosis, and makes a decision about patient disposition. The ED physician enters medical history, examination information, diagnosis, and patient disposition into the computerized patient EHR system. The ED physician enters medication and test orders into the order-entry system, and orders are automatically routed to the clinical laboratory, radiology, and pharmacy information systems. Given the above case study, construct a block diagram that shows the boundaries of the ED information system; components of the ED information system; and their relationships, system inputs, outputs, and interface with systems outside the ED information system.
